Are you ready to shape the future of community nursing in Moray? NHS Grampian is expanding our team with a new Specialist Practitioner District Nurse role and we’re looking for passionate, forward-thinking nurses who thrive in multidisciplinary teams that listen, learn, and lead with care.
You’ll be at the heart of our “Home First” approach, assessing patients in their own homes to prevent avoidable hospital admissions and support safe, timely discharge from hospital – aligned with the Scottish Government’s Discharge Without Delay initiative. As a member of the multidisciplinary team you will build trusted relationships, work collaboratively with the service Moray wide to deliver care that truly puts people first.
