East London NHS Foundation Trust is seeking an experienced Community Nurse to join the Unplanned Care Night Team as a part-time Band 6 Rapid Response Nurse. The role focuses on triage, initial assessment and urgent community care during night hours 20:00–08:00, across Bedfordshire North and South areas.

You will conduct home visits, manage end-of-life care and catheter care, coordinate with GPs and palliative services, and supervise a Health Care Assistant.

Competenze

• Registered nurse with NMC registration is essential.
• Experience in community/unplanned care is preferred.
• Ability to work night shifts and cover Bedfordshire areas.

Mansioni

• Triage and assess referrals into the Rapid Response Team.
• Provide initial history taking and clinical assessment at night.
• Deliver nursing care to housebound patients, including End of Life and catheter care.
• Coordinate with GPs and palliative services; supervise a Health Care Assistant.
