Job summary

CNWL NHS Foundation Trust is recruiting for Urgent Community Response Nurse/Paramedic in Inner London Boroughs of Kensington and Chelsea and Westminster. The role involves visiting patients at home, delivering admission avoidance care, and working as part of a skilled, supportive team in a fast-paced environment.

Band 6 registered nurses and paramedics with community or tertiary service experience are invited to apply.

Qualifications

• Experience as Band 6 nurse or paramedic in community or tertiary services.
• Ability to work as an independent practitioner and deliver home-based care.
• Willingness to work flexible hours including weekends and public holidays.

Responsibilities

• Assess, plan, implement and evaluate health care needs with patients and carers.
• Work as part of a multi-professional team with agreed areas of responsibility.
• Provide nursing care to patients in the community and develop care plans.
• Discharge patients safely or refer for ongoing assessment or rehabilitation.
• Liaise with community services and contribute to team development.
