Oxleas NHS Foundation Trust in London is seeking an experienced Adult Physical Health, Senior Staff Nurse to join our Rapid Response Team, delivering same-day urgent community care in collaboration with social care, acute trusts and primary care.

You will provide comprehensive nursing assessments, focus on falls and frailty, and coordinate care pathways across hospital and community settings as part of the Rapid Response Programme. The role is Band 6 within the Agenda for Change framework.

Formación

• Three years post registration experience / working in the community.
• Active professional registration and eligibility to work in the UK.
• Experience in community health settings and urgent care pathways.
• Ability to work with MDTs and manage complex cases.

Responsabilidades

• Provide comprehensive nursing assessments in acute and community settings.
• Develop and implement care plans with integrated community services.
• Triage urgent referrals and coordinate timely, safe patient care.
• Contribute to MDT meetings and maintain accurate records in RiO.
• Educate and supervise junior staff and support workers as required.
• Promote safety, clinical governance and evidence-based practice.
