Midlands Partnership NHS Foundation Trust is seeking a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) service in East Staffordshire. You will schedule care calls, coordinate with Locality Teams, and ensure timely referrals within discharge timescales.

The role requires flexibility, strong communication, and willingness to travel to office bases across South Staffordshire. Rotational shifts include early and late patterns.

Formación

• GCSEs or equivalent Certificates/Qualifications in Administration.
• Ability to effectively maintain, update and sustain systems.
• Demonstrate a commitment to continuous professional development.
• NVQ Level 3 in Care or equivalent qualification or experience.

Responsabilidades

• Schedule and allocate care calls on the Complex Discharge Tracker System.
• Coordinate with Home First Locality Teams to ensure timely referrals.
• Validate and update CDT records across locality teams.
• Provide face-to-face care/support to community patients as required.
