Job summary

The Midlands Partnership NHS Foundation Trust is seeking a Complex Assessment Nurse to join the Integrated Complex Assessment Team. The role centers on early review of patients transferred into assessment beds, to determine needs and funding requirements for discharge under the national framework.

You will work with multidisciplinary teams, service users and families to establish best outcomes, prioritise workload, maintain confidentiality, and meet deadlines, with travel across the D2A

Responsibilities

• The assessments of those with complex needs across all discharge to assess beds for the organisation
• Care planning and assessment supporting the MDT to establish best outcomes
• Managing own work load
• Performing early review (meet and greet) of all those transferred to assessment beds to establish needs and goals
• Working closely with external providers, individuals and their families to ensure best outcomes for all
• Palliative assessment and care planning
• Completing assessments and funding applications utilising appropriate NHS frameworks
