Job summary

HCRG Care Group is seeking a Neurology Physiotherapist to join our dedicated team across community and outpatient settings, delivering high-quality assessments, treatment, and rehabilitation. You will work in patients' homes and clinics to maximise independence and confidence, supported by a multidisciplinary team.

The role requires HCPC registration, experience in neurological physiotherapy, evidence-based practice, strong clinical reasoning, and willingness to travel between Wiltshire sites.

Responsibilities

• Deliver specialist neurological assessments, treatment plans and rehabilitation in community and outpatient settings.
• Manage a varied caseload independently with accurate documentation and outcome evaluation.
• Collaborate with multidisciplinary team on assessment, care planning and reviews.
• Promote independence and involve patients and carers in decisions.
• Support admission avoidance and timely discharge from hospital by home/outpatient care.
• Manage clinical risks and adapt interventions across settings.
• Maintain high professional standards and act as a role model.
• Use and manage therapy equipment safely and support service improvements.

We have an exciting opportunity for a Neurology Physiotherapist to join our dedicated team, supporting individuals with neurological conditions to maximise their independence, function, and quality of life. Working across community and outpatient settings, you will deliver high-quality physiotherapy assessment, treatment, and rehabilitation programmes tailored to each patient's needs. • Deliver specialist physiotherapy assessments, treatment plans and rehabilitation programmes for patients with neurological conditions in community and outpatient settings.
• Manage a varied caseload independently, maintaining accurate, timely documentation and evaluating patient outcomes in line with professional standards.
• Work closely within a multidisciplinary team, contributing to assessment, care planning, treatment reviews and coordinated patient care.
• Promote patient independence and wellbeing, ensuring patients and carers are actively involved in decisions about their care and rehabilitation goals.
• Support admission avoidance and facilitate timely discharge from hospital by providing care in the most appropriate setting.
• Manage clinical risks effectively and adapt interventions to meet patient needs within a range of community environments.
• Maintain high standards of clinical practice, acting as a professional role model and ensuring excellence in patient care.
• Use and manage therapy equipment safely and appropriately, while supporting the delivery and continuous improvement of a high-quality therapy service.
  We're looking for a compassionate and motivated Neurology Physiotherapist who is passionate about delivering high-quality rehabilitation and improving patient outcomes. To be successful in this role, you'll ideally have:
• A degree or diploma in Physiotherapy and current registration with the Health and Care Professions Council (HCPC).
• Experience in neurological physiotherapy practice and a broad range of clinical assessment and treatment skills.
• Evidence of continuing professional development and a commitment to delivering evidence-based care.
• Strong clinical reasoning, risk assessment, and decision-making skills, with the ability to recognise and respond to changes in a patient's condition.
• Excellent communication and interpersonal skills, with the ability to build positive relationships with patients, carers, and multidisciplinary colleagues.
• Strong organisational skills, the ability to manage a caseload effectively, and confidence working both independently and as part of a team.
• Good IT skills and experience maintaining accurate clinical records.
• A flexible and adaptable approach, with the ability to travel between sites across Wiltshire.
• A full UK driving licence and access to a suitably insured vehicle.
