Job summary

HCRG Care Group is seeking a Band 5 Physiotherapist to join the Integrated Neighbourhood Team. You will assess and treat patients in the community, support home visits or care homes, and contribute to person-centred care plans.

You will hold HCPC registration, be a CSP member or equivalent, and have a UK driving licence. The role offers NHS AfC Band 5 terms, ongoing CPD, and development opportunities within a supportive team.

Qualifications

  • Diploma or degree in Physiotherapy and HCPC registration.
  • Membership of the Chartered Society of Physiotherapy or equivalent.
  • Evidence of continuing professional development (CPD).
  • Full UK driving licence required.
  • Experience as a Band 5 Physiotherapist in community/ NHS settings.

Responsibilities

  • Carry out therapeutic assessment, diagnosis and treatment with patient-centred goals.
  • Maintain up-to-date records in line with professional guidelines.
  • Manage own caseload and capacity within the Integrated Neighbourhood Team.
  • Support integration of health and social care practices with joint pathways.

Job description

Are you an experienced Physiotherapist looking to take the next step in your career? We are seeking a passionate and motivated B5 Physiotherapist to join our dynamic team and play a key role in delivering high-quality, patient-centred care.

The Integrated Neighbourhood team is a community-based service which supports and works with people, either in their own home or in Residential or Nursing Homes. The aim of the service is to maximise and maintain independence based on patients identified needs through rehabilitation. Following an initial assessment and, in consultation with the service user, a care plan and rehabilitation programme is drawn up. As our services transform, there may be times you will be expected to support both nursing and therapy in the delivery of care, within an appropriate scope of practice and following relevant on-the-job training.

Main Responsibilities
  • To carry out therapeutic assessment, diagnosis and treatment with patients using clinical reasoning person centred goals, who may have diverse presentations and a range of physical and psychological conditions within the context of a community-based, multi-disciplinary team.
  • Responsibility for up-to-date record keeping within professional guidelines and HCRG Care group Record keeping policy.
  • To hold responsibility for own caseload and capacity.
  • Assist in further integration of health and social care practices, helping to break down barriers and enable more joint working and shared pathways, working with other practitioners and teams.
  • Resolve varied problems using analytical /problem solving techniques to understand the causes.
  • Receive support from the Integrated Neighbourhood Team, pro-actively identifying own support needs through reflective practice and taking on board constructive feedback ensuring continuous professional development.

Essential
  • Diploma/ Degree in Physiotherapy
  • Membership of Chartered Society of Physiotherapy and/ or evidence of other professional liability insurance to practice
  • Registered with HCPC
  • Evidence of continuing professional development (CPD) recorded in a portfolio, as recommended by HCPC
  • Full UK driving licence
  • To have gained effective experience in a range of practice areas from student placements and/or previous recent experience as a Band 5 Physiotherapist
  • To demonstrate knowledge of principles and practices within Physiotherapy to include knowledge of medical conditions prevalent in the elderly such as Parkinson’s Disease, Stroke, MS, COPD, falls, orthopaedic conditions, general ageing and diabetes
  • Good working knowledge of typical work-related systems, procedures, equipment and technology
  • Knowledge and understanding of the legislation that applies to care and health work
Desirable
  • Previous recent experience as a Band 5 Physiotherapist within a Community setting.
