Job summary

HCRG Care Group is seeking a Senior Specialist Neuro Occupational Therapist to lead neuro rehab in the Community Neurological Service. Based at Wilton Health Centre, you will assess, treat and support patients with complex neurological conditions across rehabilitation pathways.

You will mentor colleagues, support students, and drive service development with a focus on patient-centred care and evidence-based practice within a multidisciplinary team.

Qualifications

  • BSc (Hons) OT with HCPC registration and neuro rehab experience.
  • Minimum one year’s experience supporting adults with neurological conditions.
  • Proven ability to deliver patient-centred assessment, care planning and case management.

Responsibilities

  • Deliver specialist neurological OT assessment, treatment and rehabilitation in community and outpatient settings.
  • Develop and implement personalised care plans to maximise independence and quality of life.
  • Support timely hospital discharge and smooth transitions of care across services.
  • Manage complex neurological cases and provide specialist advice to teams.
  • Ensure patients are at the centre of care and involved in decisions and goal setting.
  • Assess, prescribe and train in the safe use of specialist equipment.
  • Maintain accurate clinical documentation and evaluate treatment outcomes.
  • Promote infection prevention and safe clinical practice.

Job Introduction

We are seeking an experienced and compassionate Specialist Neuro Occupational Therapist to join our Community Neurological Service, providing specialist assessment, treatment, care, and support to individuals living with complex neurological conditions. This is a rewarding opportunity to work with patients across their rehabilitation journey, helping them achieve meaningful goals and maximise their quality of life.

As a senior member of the team, you will provide expert clinical guidance and leadership, supporting both registered professionals and support workers to deliver outstanding person-centred care. You will also play a key role in developing the service through the supervision, mentoring, and professional development of colleagues and students.

This role is ideal for a motivated professional who is passionate about neurological rehabilitation, enjoys working collaboratively within multidisciplinary teams, and is committed to delivering high-quality care that has a genuine impact on patients and their families.

Key Responsibilities
  • Deliver specialist neurological occupational therapy assessment, treatment, and rehabilitation for patients in community and outpatient settings.
  • Develop and implement personalised care plans to maximise independence and quality of life.
  • Support timely hospital discharge and seamless transitions of care across acute, stroke, and community services.
  • Manage complex neurological cases, providing specialist advice and treatment recommendations to multidisciplinary teams.
  • Ensure patients are at the centre of care, actively involving them in decisions and goal setting.
  • Assess, prescribe, and provide training in the safe use of specialist equipment where required.
  • Maintain accurate clinical documentation and regularly evaluate treatment outcomes.
  • Promote infection prevention and control, ensuring safe and effective clinical practice.
Qualifications & Experience
  • BSc (Hons) in Occupational Therapy and current HCPC registration.
  • Post-registration qualification and experience in neurological rehabilitation.
  • Minimum one year's experience supporting adults with neurological conditions.
  • Proven ability to deliver patient-centred care, assessment, care planning and case management.
  • Experience working within multidisciplinary teams and alongside external agencies.
  • Demonstrable commitment to continuous professional development and evidence-based practice.
  • Experience teaching, mentoring, and supporting colleagues, students, patients and carers.
  • Knowledge of clinical audit, service improvement and quality standards.
  • Evidence of leadership skills and a flexible, innovative approach to care delivery.
