Job summary

Hertfordshire Community NHS Trust is seeking an enthusiastic Neurological Physiotherapist to join our Integrated Community Stroke and Neuro Service. You will manage a specialist caseload across East and North Hertfordshire, delivering assessment, treatment and rehabilitation in patients' homes and community settings.

You will work within a multidisciplinary team, providing expert advice, education and leadership to colleagues and students.

Qualifications

• Physiotherapy degree or equivalent.
• HPC registration.
• Experience of working with neurological patients in a community / inpatient setting.
• Documented evidence of professional development and reflective practice in neurological rehabilitation (CPD).
• Attendance at post-graduate neurological training courses (e.g., normal movement, spasticity management, splinting, cognition and perceptual assessment, fatigue management).

Job description

You will join a highly experienced team of specialist Neurological Clinicians, with excellent opportunities for collaborative and joint working. The post holder will be responsible for managing a physiotherapy caseload within the HCT Community Neuro Service as part of our Integrated Community Stroke and Neuro Service (ICSNS), providing specialist assessment and treatment for patients with a variety of neurological conditions and needs. Main duties of the job

We are looking for an enthusiastic and motivated Physiotherapist to join our Integrated Community Stroke and Neuro Service. You will manage a specialist caseload of patients with neurological conditions, providing assessment, treatment and rehabilitation within patients' homes and a variety of community settings across East and North Hertfordshire.

Working as part of a highly experienced multidisciplinary Community Neuro Team, you will deliver person-centred care for individuals with a wide range of neurological conditions and rehabilitation needs. You will work closely with health and social care colleagues, carers and families to support patients in achieving their goals and maximising their independence.

The role includes participation in multidisciplinary team meetings, supervision and support of junior staff and students, and providing specialist advice and guidance to colleagues, patients and carers. There may also be opportunities to gain experience across different areas of the service, including specialist neuro rotations.

We currently provide a six-day service and are working towards a seven-day service model. The successful candidate will be expected to participate in the weekend rota, currently working approximately one Saturday in every six.

Detailed job description and main responsibilities

Main duties of the job

We are looking for an enthusiastic and motivated Neurological Physiotherapist to join our Integrated Community Stroke and Neuro Service. You will work as part of a highly experienced multidisciplinary team, delivering specialist assessment, treatment and rehabilitation for adults with stroke and a wide range of neurological conditions.

Managing your own caseload, you will provide person-centred rehabilitation across a variety of settings, including our Neurological Rehabilitation Unit, outpatient clinics, and patients' homes across East and North Hertfordshire. You will support patients with complex neurological rehabilitation needs to maximise their independence, wellbeing and quality of life.

Working closely with patients, carers, families and health and social care colleagues, you will contribute to coordinated care planning and provide specialist advice and education to support rehabilitation outcomes. You will also play a key role in multidisciplinary team working, supervision of students and support staff, and service development activities including audit and quality improvement.

This role offers opportunities to broaden your experience across different areas of the Hertfordshire Community Neuro Service, including potential participation in specialist neuro rotations. The post holder will also participate in the service weekend rota, which currently involves approximately one Saturday in every six weeks.

Qualifications and Training

• Documented evidence of professional development and reflective practice in relevant aspects of neurological rehabilitation: e.g. attendance at in-service training sessions, reflective practice etc
• Attendance at relevant post-graduate neurological training courses e.g. Normal movement, spasticity management, splinting, cognition and perceptual assessment, fatigue management

Experience and Knowledge

• Ability and experience of carrying out a comprehensive neurological Physiotherapy assessment in order to formulate a comprehensive neurological treatment plan and goals.
• Knowledge and experience of using a wide range of therapeutic Physiotherapy interventions for the treatment of Neuroloogical patients
• Knowledge and experience of using a range of recognised outcome measures and goal setting skills as appropriate to the rehabilitation of Neurological patients
• Good understanding of current NHS issues, professional standards, CPD, clinical governance
• Experience of supervising and appraising other staff e.g. rehabilitation assistants
• Experience of supervising students.
• Experience in delivering training to Physiotherapists and other members of the MDT

Skills and Abilities

• Physical skills, dexterity, coordination and sensory skills for assessment and manual treatment of patients. A good ability with specific handling and sensory techniques specific to the treatment of patients with neurological dysfunction.
• An ability to demonstrate clinical problem solving in complex cases.
• Ability to prioritise patient workload
• Ability to carry out patient manual handling tasks.
