Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Neighbourhood Care Coordinator to manage a caseload of residents and coordinate care across primary care, community and social services. The role supports whole-person care and works with GPs, nurses and other professionals to ensure timely access to services.

The post offers flexible working options and is based across neighbourhood teams in South Bucks and the Chilterns.

Qualifications

• Experience of working in a Health or Care setting.
• Experience of coordinating patient care.
• Experience of using health or care digital systems.
• Accurate data entry and typing skills.
• Proficient in all aspects of Microsoft Office.
• Travel between sites may be required.

Responsibilities

• Act as the named Care Coordinator for a caseload of residents.
• Build positive, trusted relationships with residents and carers throughout their care journey.
• Take a whole-person approach across physical health, mental health, and social circumstances.
