Job summary

Buckinghamshire Healthcare NHS Trust is recruiting a Community Healthcare Assistant Practitioner (Band 4) to support the Wycombe Rapid Response and Intermediate Care teams, carrying a caseload of patients with acute and chronic conditions to maximise independence.

This permanent, part-time role is 30 hours weekly (8am–9pm, with weekend working). Flexible working options are offered, with NHS benefits including generous annual leave, pension schemes and discounts.

Qualifications

• Level 3 Health and Social Care Diploma or equivalent.
• Significant experience in patient-facing environment with a rehabilitation focus.
• Ability to motivate self and work with all levels of staff.

Responsibilities

• Deliver clinical and therapeutic care within set protocols.
• Perform patient assessment, plan and deliver high standards of care; develop, implement and evaluate care plans.
• Carry a caseload of non-complex patients.
• Co-ordinate admissions, transfers and discharge per Trust policies.
• Assist patients in maintaining personal hygiene with dignity and cultural sensitivity.
• Responsible for safe and competent use of equipment and mobility aids.
