Qualifications

• Evidence of continuing professional development
• Masters degree or equivalent qualification/knowledge & experience relevant to role
• Experience delivering complex change across health and social care environments

Responsibilities

• Lead development, planning and delivery of Integrated Neighbourhood Teams in Buckinghamshire
• Drive integration of services across health and social care with VCSE partners
• Ensure joined-up care and prevention is delivered across two neighbourhoods (Aylesbury and South-West)
