Job summary

Together First CIC is seeking a Neighbourhood Care Coordinator to support integrated, person-centred care across health, social care, and voluntary sectors. You will work with GP Neighbourhood Representatives, PCNs, MDTs, and community partners to coordinate care pathways for patients with complex needs.

Responsibilities include coordinating MDT meetings, managing referrals and caseloads, maintaining records, and supporting personalised care planning while promoting collaboration across agencies.

Qualifications

• Experience in health, social care, or voluntary sector environments.
• Solid admin skills, meeting coordination and minute-taking.
• Awareness of GDPR and safeguarding policies.

Responsibilities

• Coordinate MDT meetings with agendas, actions and records.
• Support patients and carers by coordinating personalised care plans.
• Manage referrals and patient caseloads while monitoring pathways and outcomes.
• Act as liaison between GPs, primary care and community services to ensure integrated care.
• Maintain accurate records using EMIS, SystmOne and other clinical systems.
