Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Neighbourhood Care Coordinator to support residents with health, wellbeing and social care needs across Buckinghamshire. The role focuses on a whole-person approach and coordinating services with health, social care and voluntary sector partners.

The post emphasises working across neighbourhood partnerships, building relationships with residents and professionals, and improving the care journey while gathering feedback for service development.

Qualifications

• GCSE or equivalent in English and Maths.
• Experience of working in a health or care setting.
• Proficient in health/care digital systems.
• Experience coordinating patient care.
• Experience using Excel for data management.
• Proficient in Microsoft Office.

Responsibilities

• Act as the named Care Coordinator for a caseload of residents.
• Build positive, trusted relationships with residents and carers throughout their care journey.
• Take a whole-person approach by considering physical health, mental health and social circumstances.
• Support residents to access services across primary care, community health, mental health, social care, housing and voluntary organisations.
• Maintain regular contact with residents to ensure care plans progress and new needs are identified.
• Coordinate with GPs, nurses, social workers and other professionals to ensure joined-up care.
• Help residents understand conditions and prepare for appointments to improve confidence in self-management.
• Collect and record patient feedback and outcome measures to improve services.
• Maintain records and manage referrals in line with information governance and coordinate multidisciplinary meetings.
