Job summary

Responsibilities

• Contribute to processes for proactively case finding very high intensity community patients
• Assess physical and psycho-social needs of caseload and initiate therapeutic treatments
• Improve health outcomes using specialist nursing skills
• Manage referrals and develop case management programmes
• Provide caseload management for long-term conditions
• Coordinate and evaluate care using the single assessment process
