Job summary

Buckinghamshire Council is seeking a Social Care Assistant to support residents in the community, including those recently discharged from hospital or with disabilities. You will help individuals regain independence and manage daily routines, providing high-quality direct care aligned with person-centred goals.

The role involves flexible shift patterns (7am–10pm, seven days a week) and a requirement for a full UK driving licence with a vehicle.

Qualifications

• Must be able to deliver personal care with dignity and respect.
• Excellent communication and listening skills are essential.
• Willingness to work variable shifts including weekends.

Responsibilities

• Provide information and advice on care and support options.
• Deliver care in clients' homes in a safe and compliant manner.
• Ensure plans meet individual care outcomes and enable access to services.
• Maintain safeguarding and escalate concerns as needed.
• Support coordination with carers, service providers and professionals.
• Assist with personal care tasks and everyday activities.
