Job summary

The Pennine Care NHS Foundation Trust is recruiting for a Band 5/6 Learning Disability Nurse within the Crisis Admission Avoidance Service, supporting young people aged 16-25 with learning disabilities and complex care needs. You will work in a multidisciplinary team to assess needs, coordinate care and develop person-centred plans to prevent unnecessary hospital admissions.
