Job summary

Pennine Care NHS Foundation Trust is seeking a Band 5/6 nurse for the Crisis Admission Avoidance Service supporting 16–25-year-olds with learning disabilities and complex care needs. You will work within a MDT to assess needs, plan care and coordinate services to prevent hospital admissions and promote positive outcomes.

The post offers development through training, supervision and a clear progression pathway, with opportunities to work across Radcliffe Place Service and other LD hubs within

Qualifications

• Experience of working with children and adults with learning disabilities.
• Ability to work in a multidisciplinary team and coordinate care.
• Experience with risk assessments and care planning.
• NMC registered nurse with current registration.

Responsibilities

• Assess care needs and develop, implement and evaluate care plans.
• Coordinate care with service users, families, carers and partner agencies.
• Support supervision and development of junior staff.
• Maintain accurate records per policies and guidelines.
