Job summary

The Pennine Care NHS Foundation Trust invites applications for a Crisis Admission Avoidance role supporting young people aged 16-25 with learning disabilities and complex care needs. You will work with a multidisciplinary team to assess needs, develop person-centred care plans and coordinate care to prevent hospital admissions.

Based at the Radcliffe Place Service with potential cross-site duties, the post offers development through supervision, mentorship and training.

• Will work within a multidisciplinary team to assess needs and plan care.
• Involves health assessments, risk assessments and person-centred care planning.
• Opportunities for professional development through supervision and mentorship.

Responsibilities

• Undertake comprehensive assessments and contribute to health and risk assessments.
• Develop, implement and review care plans and communication profiles.
• Coordinate referrals and triage to ensure timely access to services.
• Provide supervision and support to junior staff and contribute to workforce development.
