Job summary

Pennine Care NHS Foundation Trust in Greater Manchester is seeking a Band 5 Community Learning Disability Nurse to work with individuals with learning disabilities and complex needs in the community. You will deliver specialist pathways and person-centred care as part of a supportive multidisciplinary team.

You will manage your own caseload, undertake specialist health and behavioural pathways, and work in a range of settings including patients' homes.

Qualifications

• Demonstrates professional curiosity and safeguarding.
• Motivated to develop specialist skills in learning disability nursing.

Responsibilities

• Manage own caseload within the community.
• Deliver specialist pathways and evidence-based interventions.
• Promote health and wellbeing, reducing health inequalities.
• Work with families, carers and partner agencies.
• Contribute to specialist pathways (Positive Behaviour Support, Autism, Epilepsy, Dysphagia, Dementia).
