Pennine Care NHS Foundation Trust is seeking a Named Nurse for Learning Disabilities to join the Crisis Admission Avoidance Service in Greater Manchester. You will work within a multidisciplinary team to deliver person-centred care for young people aged 16-25 with learning disabilities and complex needs.

The post supports assessments, risk management, care planning, and collaboration with families, carers and partner agencies. A development pathway from Band 5 to Band 6 is available.

Qualifications

• Applications are welcomed from newly qualified and experienced Band 5 practitioners.
• Competency-based progression to Band 6 through a structured development pathway.
• Undertake comprehensive assessments, support risk management and provide person-centred care planning.
• Collaborate with service users, families, carers and partner agencies to coordinate effective care.

Responsabilités

• Support junior staff and contribute to workforce development and supervision.
