Job summary

Pennine Care NHS FT is recruiting a Band 5 Community Learning Disability Nurse to support people with learning disabilities across Greater Manchester. You will deliver specialist pathways, assessments and person‑centred care within a multidisciplinary team.

You will manage your own caseload in the community, promote health and equality, and work with families, carers, primary care and social care to improve health outcomes for individuals with learning disabilities.

Qualifications

• Registered RNLD
• Previous LD nursing experience
• Current UK professional registration is required
• Desirable: previous LD community nursing experience

Responsibilities

• Managing your own caseload within the community.
• Undertaking specialist health and behavioural pathways, assessments.
• Supporting individuals in a range of settings, including their own homes.
• Promoting health, wellbeing and reducing health inequalities for people with learning disabilities.
• Working closely with families, carers, primary care, social care and partner agencies.
• Contributing to specialist pathways including Positive Behaviour Support, Physical Health, Autism, Epilepsy, Dysphagia and Dementia.
