Job summary

Hertfordshire Partnership University NHS Foundation Trust is recruiting for a role within the Physical Health Team, delivering physical health checks and wellbeing assessments to people with severe mental illness. You will travel between sites and in the community, with a requirement for a valid UK driving licence.

Qualifications

• NVQ/Mental Health Certificate Level 2 or Care Certificate on appointment.
• GCE/GCSE Maths and English.
• Venepuncture competent.
• Car driver (unless disability defined by Equality Act prevents driving).
• Commitment to Continuing Personal Development.

Responsibilities

• Contribute to assessment of physical and mental health needs for people with severe mental illness.
• Work with agencies to deliver a systems approach to meet health needs.
• Carry out physical health checks (BP, pulse, weight, BMI) and venipuncture as required.
• Support implementation of new digital tools for health assessments.
• Promote healthy lifestyles and signpost to relevant services.
• Maintain and review SMI registers and assist in outreach health assessments.
• Engage in regular supervision, training and personal development plans.
