Essex Partnership University NHS Foundation Trust is looking for a post holder to support older adult mental health, dementia and frailty services in Basildon and Brentwood. You will work with a multidisciplinary team to address both mental and physical health needs, promoting recovery and person-centred care.

The role involves physical health checks, health promotion, and coordinating care plans with GPs and staff to optimise patient outcomes within the recovery model.

Mansioni

• Work with other agencies to deliver a systems approach in ensuring patients mental and physical health wellbeing needs are met.
• The post holder will undertake annual physical health checks of patients identified as having severe mental illness, taking and recording blood pressure, pulse, weight, BMI, and venepuncture as required.
• Arrange physical health assessments with service users at home, in clinics, and at GP surgeries; promote annual physical health reviews for patients with SMI.
• Provide information on health promotion including healthy eating, sport and exercise and smoking cessation.
• Develop care plans with patients and staff and complete the electronic physical health assessment form on the electronic health record; share information with patients and GPs.
• Promote positive mental and physical health to patients, carers, families and staff to help patients identify their own physical health goals and monitor progress with the team.
