Job summary

LOROS The Leicestershire & Rutland Hospice is seeking a Nurse Specialist to lead holistic MND care plans. You will perform home visits, deliver expert interventions, and support patients and carers in collaboration with the MDT.

The role involves telephone advice, nurse-led clinics, and coordination with other services while maintaining excellent records and compliance with policies. Part-time, pro-rata salary reflects 22.5 hours per week.

Qualifications

• Registered nurse with NMC registration and current practice.
• Experience in managing complex care needs and planning.
• Strong communication with patients, carers and multidisciplinary teams.

Responsibilities

• Undertake home visits and holistic need assessments.
• Provide specialist intervention to support patients and carers.
• Act as resource for care teams and external organisations on MND care.
• Provide telephone advice to patients, carers and professionals; plan care accordingly.
• Manage nurse-led MDT meetings and MND clinics.
• Maintain records and ensure compliance with policies and data protection.
