Job summary

LOROS Hospice in Leicester is seeking a Nurse Specialist to join the MDT caring for patients with motor neurone disease. You will support patients and carers, provide holistic assessments and lead nurse-led clinics and MDT meetings.

Role involves home visits, telephone advice, collaboration with primary care and voluntary organisations, and contributing to ongoing education, audits and service development at LOROS.

Responsibilities

• Undertake home visits and complete holistic specialist needs assessments.
• Provide specialist intervention to support patients and carers.
• Act as a resource for primary health care teams on issues relating to MND care.
• Provide a telephone advice service to patients, carers and professionals.
• Manage nurse led MDT meetings and undertake nurse led MND clinics.
• Liaise with Hospice staff and refer to MNDA and other agencies.
• Monitor progress of all MND patients and support informed choices regarding care.
• Comply with NMC code and Hospice policies and procedures.
• Participate in mandatory training and personal development.
