Job summary

The Myton Hospices in Coventry is seeking a Registered Nurse or Allied Health Professional to join our new MND Care Coordinator role. You will work across Coventry and Warwickshire, providing equitable, specialist care for people with Motor Neurone Disease and supporting families.

Travel to patients' homes and partners will be required. Working with the Care Coordinator at Mary Ann Evans Hospice and regional teams, you will manage referrals, assess needs, coordinate care pathways, and

Qualifications

Registration with a relevant professional body (NMC/HCPC) is required.
Post-registration clinical experience in neurology, palliative care, or community nursing is preferred.
Experience collaborating across healthcare, social care and voluntary sectors is essential.

Responsibilities

Support people with MND and their families to manage symptoms.
Holistic needs assessment and management planning from diagnosis to end of life.
Receive, make and coordinate referrals.
Develop and coordinate seamless specialist care pathways between external and internal stakeholders.
Establish and provide an advisory service and a single point of contact for patients.
Monitor outcomes and reporting.
Lead service development and improvement.
