Job description
Overview

As an integral member of the team, you will provide specialist expertise in the use of medicines while supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients to ensure an integrated approach to the management of medicines. The post holder may be required to travel across the different sites within the Trust where we have bedded services inpatients, so some flexibility is required. Responsibilities
  • Conduct structured medication reviews for patients with polypharmacy, long term conditions or high risk patients to ensure integrated management of medicines.
  • Collaborate with the existing team of Band 7 pharmacists and be supported by Senior Clinical Pharmacists to gain skills in each rotation.
  • Develop strategic, quality improvement and leadership skills within the role.
  • Travel across Trust sites with Bedded Services inpatients as required to deliver clinical pharmacy services.
About the Role and Opportunities

An exciting opportunity has arisen for dynamic, motivated and innovative pharmacists to join our expanding Medicines Management team at Central London Community Healthcare Trust. We are looking for a full time Pharmacist to provide a clinical pharmacy service to our inpatients in our Bedded Services Rehabilitation wards across the Trust.
