A community health service provider in Westminster is seeking a full-time Pharmacist to join its Medicines Management team. As an integral member of the team, you will provide specialist expertise in the use of medicines while supporting to address both public health and social care needs of patients. You will be involved in conducting structured medication reviews for patients with polypharmacy, long term conditions or high risk patients to ensure an integrated approach to the management of medicines. The post holder may be required to travel across the different sites within the Trust where we have bedded services inpatients, so some flexibility is required.
