Job summary

Dimensions is seeking a Lead Support Worker in Ash Vale (GU16) to lead a small team and support five adults with learning disabilities. You will coach staff, deliver person-centred care, and work with locality management to drive positive outcomes.

You will use Activate and Active Support approaches to empower individuals, help them build independence and develop new skills, while ensuring high-quality daily support and a positive team culture.

Responsibilities

• Lead a team to deliver person-centred care for five adults with learning disabilities
• Coach and mentor support workers, sharing good practice with the team
• Contribute to reviews, continuous improvement and regulatory standards
• Support daily routines (personal care, medication, meals, shopping, home upkeep)
• Use Activate and Active Support to set meaningful goals and measure progress
• Build strong relationships with families, circles of support and colleagues
