Job summary

Praxis Care is hiring a Team Leader to oversee a team of Support Workers at Kilmorey House. The role focuses on coordinating person-centred support, designing individual programmes, and ensuring safe service transition for individuals.

The Team Leader will supervise staff, manage time/attendance, and participate in appraisals, with a strong emphasis on dignity, respect, and independence for those served. Sleep-in payments apply where required.

Qualifications

• 2 years’ experience in a health and social care setting and commitment to undertake a professional qualification
• Level 2 Safe and Effective Care Practice Diploma or registration with NISCC/NMC or Allied Health Professional
• Qualifications in Social Work, First Level Nursing, Allied Health Profession, or Level 5 Diploma in Leadership & Management for Health and Social Care
• Psychology Degree with 1 year paid experience in HSC setting and commitment to ongoing qualification

Responsibilities

• Plan, coordinate and implement the safe transition of new individuals into the service and community
• Liaise with families and multidisciplinary teams to promote needs
• Develop and implement individual and group support programmes to maximise independence
• Coordinate ongoing assessments and reviews using suitable models
• Provide leadership and supervision to Support Workers and manage attendance and procedures
• Ensure records are created and maintained in line with policy and statutory requirements
• Support regulatory compliance with relevant bodies
