Job summary

Praxis Care is seeking a Team Leader for Beechcote Supported Living in Portadown. The role supports individuals with practical, physical and emotional needs, using a key worker system to design and coordinate individual and group programmes with Support Workers.

Salary based on a 39-hour FTE and pro rata to contracted hours; sleep-ins may be required with an additional £45 per night payment.

Qualifications

• Participate in appraisal and staff review processes.
• Adhere to company values, behaviours, and competencies.
• Engage in supervision as required.
• Carry out duties per Praxis expectations, regulations and policies.
• Take on additional duties as necessary.
• Work hours to fit service needs.

Responsibilities

• Plan, coordinate and implement safe transition of new individuals into the service and community.
• Liaise with families and multidisciplinary teams to promote needs.
• Plan, develop and implement individual and group support programmes.
• Coordinate ongoing assessment and reviews using suitable models.
• Support the physical, emotional, and personal needs of individuals with respect and dignity.
• Ensure medication storage and administration per policy and law.
• Provide leadership, supervision, and management to Support Workers.
• Participate in learning and development within the organization.
• Maintain records and reports in line with policy and statutory requirements.
• Support regulatory compliance with regulators.
