Job summary

Practice Plus Group is hiring an Integrated Care Nurse to join our Primary Care team at HMP Pentonville in London. This role offers 37.5 hour weeks, with a 4-shift pattern, and opportunities to develop a broad range of clinical skills including long-term conditions, wound care, and vaccination training, within a supportive multi-disciplinary prison healthcare setting.

Responsibilities

• Work within a multi-disciplinary team on inpatient settings for mental and physical health.
• Undertake medication rounds, dispense controlled medication, attend emergency responses, assess patients and undertake reception screening.
• Deliver nurse-led clinics to your patient group with support from a multi-disciplinary team.
• Provide outstanding evidence-based care, listening to and acting on feedback to personalise care.
• Deliver non-judgemental care that makes a real difference.
• Inspire excellence by forging strong links with partners and stakeholders.
• Promote best practice, clinical supervision, evidence-based care and continuous quality improvement.
• Share knowledge to ensure safe care and build strong multi-disciplinary teams.

About The Role

We have an exciting opportunity for an Intergrated Care Nurse to join our Primary Care team at HMP Pentonville, a category B Prison located in London

You'll also have the opportunity to build a broad range of clinical skills through varied and rewarding training, including long-term health conditions, wound care, vaccination training, and much more.

Hours

37.5 hrs

4 Shifts of 7.30am - 17.30pm inclusive of weekends and bank holidays

Salary

This position is full time; you'll receive an annual salary up to £55086 per annum

What you'll be doing

As a Intergrated care nurse your responsibilities will vary. You will

• Work within a multi-disciplinary team primarily on our inpatient that cares for both mental health and physical health.
• Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
• Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
• Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
• Deliver non-judgemental care that makes a real difference
• Inspire excellence by forging strong links with our partners and stakeholders
• Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
• Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.
