Job summary

Center for Life Management is seeking an Adult Community Case Manager to provide care planning, resource coordination, and community-based support for adults 18+. The role emphasizes collaborative care with internal programs and external partners, and includes home-based counseling, crisis intervention, and documentation in line with agency standards.

This is a full-time position averaging 40 hours per week with FLSA non-exempt status.

Qualifications

• Bachelor's degree in a health-related field preferred
• Two years of direct-service experience preferred
• Case management or coordination experience preferred
• Ability to exercise clinical judgment in routine to emergent situations
• Proficient with Microsoft Office applications
• Ability to document effectively in clinical work
• Ability to navigate an electronic health record
• Strong communication and organizational skills
• Ability to work as part of a clinical team and be self-directed
• Willingness to acquire and practice new clinical skills

Responsibilities

• Provide assessments to create care plans and set objectives
• Assist clients with applying for and maintaining medical coverage
• Provide outreach, coordination, and linkage to supports
• Counsel clients in home or community settings
• Develop coping strategies to reduce symptoms
• Provide family training and counseling when appropriate
• Deliver psychoeducation including IMR as needed
• Provide crisis intervention and ensure safety
• Document services in accordance with policies
• Collaborate with treatment teams for wrap-around care
• Maintain professional conduct and confidentiality
• Participate in committees as assigned
