Wye Valley NHS Trust is seeking a Band 4 Discharge Co-ordinator to help plan safe, timely patient discharges. The role involves liaising with ward staff, community services, patients and families to prevent delays and representing the team at daily ward board rounds and twice-daily MDT meetings.

The post requires a Level 4 health or social care qualification or extensive hospital discharge experience, with autonomous working and strong communication skills in a busy hospital discharge setting.

Formación

• NVQ Level 4 or Foundation Degree in health or social care, or extensive hospital discharge experience.
• Completion of the Care Certificate.
• GCSE English and Maths, or equivalent.

Responsabilidades

• Plan patient discharges, supporting decisions with the Integrated Discharge Team, ward MDT and community partners.
• Represent the Integrated Discharge Team at ward daily board rounds.
• Attend twice-daily ward MDT meetings, working to the 'Why not Home, Why not Today' approach.
• Communicate discharge actions effectively and promptly.
• Communicate with patients, relatives and community services to keep discharge planning proactive and continuous.
• Liaise with patients and families about discharge.
• Carry out delegated duties without direct supervision and report back to the ward team and case manager.
