Job summary

Barking Havering and Redbridge University Hospitals NHS Trust is seeking a Band 4 Discharge Co-ordinator to join our Hospital Complex Discharge Team. You will support inpatient wards to plan safe, timely discharges across Queen's Hospital and King George Hospital, coordinating actions with MDTs and community partners.

The role requires excellent communication, organisational skills and the ability to prioritise workload in a fast-paced environment.

Qualifications

• Experience in a patient-focused environment is essential.
• Good communication and organisational skills.
• Ability to prioritise workload and work across multidisciplinary teams.

Responsibilities

• Coordinate and plan safe, timely discharges across hospital sites.
• Identify discharge barriers and work with the MDT to resolve them.
• Attend clinical board rounds and monitor Estimated Discharge Dates (EDD).
• Support patients, families and carers to understand the discharge planning process.
