The Integrated Discharge Team at Luton & Dunstable Hospital plays a pivotal role in safe, timely discharges and supports referrals to community services, with a commitment to flexible working patterns and equitable recruitment.

Qualifications

• Registered nurse with current NMC registration and NHS experience.
• Experience in community or hospital settings with terminal illness knowledge.
• Proficient in IT including Word, emails and System One.

Responsibilities

• Work with multi-disciplinary teams in hospital and community settings to ensure safe discharge.
• Communicate clearly with patients, families, carers and staff.
• Prescribe equipment to promote patient safety in the community.
• Manage GP Liaison referrals and triage.
• Provide clinical supervision with regular 1:1 sessions.
• Monitor budgets and resources.
