Job summary

East of England Community Health and Care NHS Trust in Cambridge is seeking an Integrated District Nursing Sister to join the Integrated District Nursing Service in Luton. The role is Band 6 and focuses on caseload management within an integrated team dedicated to high-quality community care.

The successful candidate will develop Integrated Community Nursing in Luton, hold a UK driving licence and have access to a vehicle. Service operates 8am–6pm, 7 days a week on a rota.

Qualifications

• Educated to degree level or equivalent e.g. Community Specialist Practitioner /PgDip.
• Independent Nurse Prescribing or willingness to undertake.
• Evidence of CPD.
• MSPP or equivalent.

Responsibilities

• Support the provision of Community Nursing care within the Integrated Community Nursing Team, providing scheduled and unscheduled care to the house bound population of Luton.
• Manage a defined caseload of patients.
• Prevent avoidable hospital admissions by holistic assessment, care planning, implementation and evaluation of patient care packages.
• Provide clinical leadership within the Community Nursing Service.
• Organise and coordinate the overall care to meet clients' care needs, possibly complex and involving other professionals and agencies.
