Job summary

Qualifications

• Evidence of CPD and ongoing professional development.
• Desirable post graduate study and experience in community nursing.

Responsibilities

• Provide scheduled and unscheduled community nursing care to housebound residents in Luton.
• Manage a defined caseload and coordinate care with a multi‑disciplinary team.
• Provide clinical leadership within Integrated Community Nursing Service.
• Assess needs, plan and evaluate care to prevent hospital admissions and support home living.
