Job summary

The role offers the opportunity to contribute to the development of Integrated Community Nursing in Luton, requires a UK driving licence and access to a vehicle, and operates on a 8am–6pm, 7 days a week rota.

• Experience in community nursing
• Dynamic, enthusiastic nursing professional

Responsibilities

• Support provision of Community Nursing care for housebound population in Luton
• Manage a defined caseload of patients
• Provide holistic assessment, care planning, implementation and evaluation to prevent avoidable hospital admissions
• Provide clinical leadership within the Community Nursing Service
• Coordinate care involving other professionals and agencies in an integrated, coordinated way
