Job summary

Mid and South Essex NHS Foundation Trust in Chelmsford is seeking an experienced IDT Nurse Assessor (Band 6) for maternity leave cover until August 2027. The role focuses on discharging patients safely from hospital to home or onward care, coordinating assessments and referrals with community partners.

You'll work across multidisciplinary teams, supporting patients with complex needs, ensuring timely, person-centred discharge planning and smooth transfers of care within Broomfield Hospital.

Qualifications

• NMC Registration is required.
• Evidence of proactive and continuous personal and professional development.

Responsibilities

• Work with patients and families with complex needs to support timely, safe discharge.
• Provide professional guidance to patients and carers throughout discharge planning.
• Develop and coordinate holistic discharge plans with multi-agency colleagues.
• Ensure discharge interventions are coordinated and not duplicated.
• Support patients with complex healthcare needs including long-term planning.
• Anticipate and coordinate health and social care needs for high-risk patients.
• Work with professionals across hospital and community services to plan care transitions.
• Act as liaison between hospital and community services, including carers.
• Facilitate smooth transfers of care from hospital to home and community settings.
• Contribute to improved patient experience and discharge coordination.
