Job summary

Plus Dane is recruiting a Floating Support Advisor to deliver person-centred support to vulnerable people, helping them live independently and sustain their tenancies across Cheshire, including Congleton.

You will assess needs, develop tailored support plans, work with partner agencies and maintain accurate records. A full UK driving licence and access to a vehicle are essential, with NVQ Level 2–4 in Social Care or equivalent and strong organisational and communication skills.

Qualifications

• Experience in supported housing or similar settings.
• Experience partnering with external agencies.
• Strong organisation and ability to manage priorities.
• Ability to maintain clear written records and documentation.
• Good IT skills and confidence with systems.
• Strong communication and customer-focused approach.
• Full UK driving licence with access to a vehicle.
• NVQ Level 2–4 in Social Care or relevant qualification.

Responsibilities

• Managing referrals, completing assessments, and signing up new clients.
• Developing and delivering individual support plans tailored to customer needs.
• Supporting customers to build independence and reduce reliance on services.
• Working with external agencies to improve outcomes and prevent homelessness.
• Reviewing support plans and ensuring services meet expectations.
• Maintaining accurate records and managing case documentation.
• Contributing to service development and improvement activities.
• Acting as a key contact for colleagues regarding external services.
