Job summary

Care Dorset Ltd is seeking a Community Reablement Worker to provide in-home reablement support and personal care, helping individuals regain independence and improve quality of life. You will work within a planned care package, promoting choice and dignity while coordinating with therapists and social care professionals.

Qualifications

• Experience delivering personal care and reablement services.
• NVQ Level 2 or 3 in Health and Social Care or willingness to work towards it.
• Experience assessing individual client needs and planning support.
• Ability to deliver care in a person-centred manner with dignity and respect.
• Strong communication skills with service users, families and colleagues.

Responsibilities

• Provide reablement support in clients' own homes to help regain independence.
• Assist with personal care, mobility, meal preparation and daily living activities.
• Develop and implement person-centred care plans with regular reviews.
• Collaborate with occupational therapists, nurses and social workers.
• Monitor progress and document wellbeing, reporting changes to the manager.
• Safeguard individuals and follow policies and procedures; maintain safety.
