Job summary

Waythrough is hiring a Support Coordinator in Bradford to deliver person‑centred floating support and housing management for adults with multiple disadvantage. You will work in clients' homes and in the community to promote independent living and wellbeing.

You will coordinate with partner agencies, manage caseloads and assess risks while maintaining accurate records. A driver with access to a vehicle is preferred, and evening/weekend work may be required.

Qualifications

• NVQ Level 3 in Health & Social Care or willingness to work towards it.
• Knowledge of welfare benefits and housing options.
• Experience with safeguarding and multi‑agency working.
• Ability to manage a caseload and keep accurate records.
• Good communication and use of case management systems.
• Driver with a vehicle for work is advantageous.

Responsibilities

• Provide high quality, person‑centred floating support and housing management.
• Support adults to sustain accommodation and wellbeing.
• Collaborate with partner agencies to coordinate support.
• Maintain case notes and data in management systems.
• Attend team meetings and flex across sites including evenings/weekends.
