Job summary

Lincolnshire Community Health Services NHS Trust is seeking a Promoting Independence Therapy Assistant- Louth on a permanent basis. The role focuses on providing rehabilitation and reablement for patients at home after hospital discharge, aiming to promote independence and assess future care needs.

The post is Band 3, full or part-time with flexible working, 37.5 hours per week, and salary £25,760–£27,476 per annum (pro rata). Closing date 15 October 2026.

Qualifications

• 5 GCSEs at grade 4 or above, or equivalent.
• A L3 Vocational Healthcare Qualification or Equivalent.
• Moving and handling training/experience.

Responsibilities

• Provide rehabilitation and reablement to patients immediately following hospital discharge in their home environment.
• Promote independence and contribute to the assessments required to determine a person's future needs.
