Job summary

Wight Healthcare is seeking a Rehabilitation Assistant to join our Community Therapy Team covering Petersfield and Bordon. You will work with Occupational Therapists, Physiotherapists and other healthcare professionals to support patients in their homes and community settings, delivering delegated rehabilitation programmes and monitoring progress to help patients regain independence.

The role emphasizes a compassionate, person-centred approach, with supervision and development opportunities

Qualifications

• Experience in health, social care or rehabilitation settings is desirable.
• Full UK driving licence and access to a vehicle for work purposes is essential.

Responsibilities

• Deliver delegated rehabilitation programmes and support mobility and functional activities.
• Support patients in their homes and community settings and monitor progress to achieve positive outcomes.
• Work alongside Occupational Therapists, Physiotherapists and other healthcare professionals in a multidisciplinary team.
