Hampshire and Isle of Wight Healthcare NHS Foundation Trust invites applications for a Reablement Assessor to join the Enhanced Recovery Service. You will conduct strengths-based home assessments, set goals and develop care plans that promote recovery, independence and wellbeing.

Working with patients, families and partners, you will support timely discharge and patient flow across health and social care. A vehicle for work is essential.

Qualifications

• Competent PC skills and evidence of previous experience using Microsoft software including Word, Excel, and Power Point and CM2000 or similar care call allocation.
• Demonstrate an acceptable level of numeracy and literacy.
• Understanding of Health and Safety and relevant NHS policies.

Responsabilités

• Manage the re-enablement care caseload, allocating work appropriately and efficiently.
• Create working rosters in response to service demands using the CM2000 system.
• Maintain excellent communication with patients, relatives and team regarding re-enablement care.
• Clear and timely communication with ERS@Home team, ICT's, Therapy teams and the IDS team in QAH and other agencies.
• Maintain clear, concise and legible documentation in accordance with organisational policy.
• Communicate on complex and sensitive issues.
• Communicate appropriate information to and from other departments/service areas as required.
• Verify consent in accordance with organisational policy.
• Facilitate and provide concise handovers to other members of the team.
• Documentation must be clear, concise, legible and signed.
• Identify and report to a registered practitioner clinical risk issues that impact on the patient/client's health or social care needs.
• Practice corporate values and contribute to team and departmental objectives.
• Attend staff meetings and contribute to the co-ordination and development of services as required.
