Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking caring Healthcare Support Workers to join the Enhanced Recovery Service. You will support people in their own homes after illness or hospital admission, helping them regain independence and daily living skills.

Working with a multidisciplinary team, you will provide person-centred reablement, communicate with patients and families, and follow care plans under supervision.

Responsibilities

• Support delivery of responsive community-based care to keep people safely in their homes.
• Contribute to intermediate care service aiding timely hospital discharge and rehab.
• Provide reablement support in line with individual care plans and goals.
