Job summary

VN Appointments in the United Kingdom is recruiting two Reablement Care Workers (Community Carers) to support individuals in their own homes, helping them regain confidence and independence after illness or hospital stays. This role focuses on enabling people to do things for themselves rather than doing everything, with a 4-week rolling rota and early/late shifts across a borough.

There are two positions: 21 hours and 35 hours per week, with every other weekend work and travel between service

Qualifications

• Previous care experience is advantageous.
• Access to a vehicle insured for business use.
• Flexibility to work a rolling shift including every other weekend.
• Ability to work in home care following care plans.

Responsibilities

• Provide person-centred care in clients' homes.
• Assist with personal care and daily routines.
• Support meals, drinks and everyday activities.
• Assist with medication routines per care plans.
• Encourage mobility and independence.
• Monitor progress and report changes to the team.
• Collaborate with therapists, nurses and social workers.
• Uphold dignity, choice and respect.
