Job summary

Buckinghamshire Healthcare NHS Trust is seeking a part-time Band 6 practitioner for the Rapid Response & Intermediate Care (RRIC) team based at Amersham Hospital. The role involves triage, specialist assessment, and developing care plans to promote independence and reduce hospital stays.

You will work within a multi-disciplinary team including Physiotherapists, Occupational Therapists, Nurses, Paramedics and Pharmacists.

Qualifications

• Registered Healthcare Professional with HCPC or RCN registration.
• Degree/ Diploma in Physiotherapy, Occupational Therapy, Nursing or Paramedic Science.
• Experience with Continuous Professional Development.
• Post graduate clinical training e.g. Advanced History Taking & Assessment.

Responsibilities

• Triage and comprehensive specialist assessment of patients and carers.
• Plan programmes of care to promote health gain and maximize independence.
• Allocate workloads to the RRIC team members.
• Collaborate with multi-disciplinary teams to ensure timely care delivery and care planning.
